Student Application - OPSB Schools - Please print form, complete, and take to school during school hours or deliver to OPSB Central Administrative Offices Monday Through Friday 9AM to Noon or 1PM to 3PM,   If you have scan capability, you may scan and attach to e-mail.  Please contact school to determine appropriate e-mail address.
